 Person Centred Planning Image

Person Centred Approaches is about how we do things. It means listening to what people say and making sure that people with learning disabilities have the lives that they want.

Person Centred Planning is making use of planning tools that help us to get to know and understand the person better.

Circle of Support image Circles of Support
A circle of support is a group of people who meet together on a regular basis to support people to achieve the things they want to achieve in life. The person is the centre of the circle and is in charge of who is part of the circle. Anybody can be part of a circle of support - the important thing is that they care enough about the person concerned to give their time, energy and commitment to helping them overcome the obstacles that are stopping them achieving their goals in life

PATH is a creative planning tool which starts in the future and works backwards to an outcome of first (beginning) steps that are possible and positive.
