The Family Pack is a set of materials to help families to co-ordinate information and services. It also helps professionals to plan the services they provide.The Family File is an important part of the Family Pack. It is an easy way for families to share basic facts and information about their child and family situation with other people. 'What Kind Of Future? Supporting young people with Down's syndrome to lead full lives after they leave school'
This booklet is for young people and their families, friends and supporters to read and talk about together. It is written in plain English and there is an easy read summary. At the end of each section there are ideas for young people and information about some easy read websites and booklets. There is a final section on useful addresses and information for families.

Action for Young Carers - is a service provided in South and East Lincolnshire by the Carers Federation Limited, Supporting young carers and their families. The poject is aimed at children and young people under the age of 18. The poject also delivers a bespoke service for carers aged 18 to 25 years called AYC Plus. Young Carers are children and young people who help look after someone at home. The person may be a parent, sibling, grandparent or other relative and may be affected by
- Mental health
- Drugs or alcohol misuse
- A physical disability
- A learning disability
- Any other long term illness or condition
Support workers work with young carers to provide an opportunity to discuss how they're feeling and work through any issues they may be facing. They also
- Provide telephone and face-to-face support together with information about other services thay may be useful.
- They can also represent young carers and their families at meetings with other professionals if the young carer wants this.

Independent Parental Supporters Independent Parental Supporters are volunteers who give impartial support to the parents of children with special educational needs.These volunteers support parents by helping them to consider their options and express their views giving them information, helping them through the assessment process.
